Need a Ride to Your Medical Appointment?

The Yavapai-Apache Nation Community Health Representative (CHR) Department provides medical transportation for community members who need a ride to their medical appointments in the Phoenix, Flagstaff, Prescott, and Verde Valley areas.

This non-emergency service is available to those who live within or up to 20 miles from the Nation’s boundaries and have no other means of transportation.

To help us serve everyone, please make your transportation requests by 5:00 p.m. on Wednesdays for appointments scheduled the following week. The CHR team prepares the schedule each Friday, and spots fill up quickly.
