Alcohol & Substance Abuse Program Needs Assessment Survey
The Yavapai-Apache Nation Alcohol & Substance Abuse Program is seeking community input through its Community Needs Assessment Survey.
The assessment will help the program better understand current community needs, concerns, and priorities and help guide future prevention, education, treatment, recovery, outreach, and support efforts.
Community members are encouraged to participate. The survey takes approximately 7–10 minutes to complete, and responses are requested by September 18, 2026.

Paper copies are available at the following locations:
- Administration – Middle Verde
- DSS Building – Middle Verde
- Tribal Housing – Tunlii
- Community Development – Camp Verde
- Senior Center – Clarkdale
Completed paper surveys may be returned according to the submission instructions included on the survey.
Fillable electronic copies may be accessed here and be submitted by email to Glory Lewis at glorylewis@yan-tribe.org.
