Diabetes Program – Medical Equipment Services (2026)

The Yavapai-Apache Nation Diabetes Program is offering limited funding assistance for approved medical equipment and supplies for Native American community members diagnosed with diabetes, while funding is available.

Services may include, but are not limited to:

• Bath benches

• Walkers

• Wheelchairs

• Blood pressure monitors

• Diabetes shoes and socks (physician referral required)

• Eyeglasses (funded up to $250 for basic frames and lenses; voucher required)

Please note:

• Services are limited and eligibility requirements apply

• No out-of-pocket expense reimbursements are allowed

• Motorized equipment cannot be rented or purchased

• Services for eyeglasses provided within the prior year are not eligible for assistance
