The Yavapai-Apache Nation Enrollment Department would like to remind all Tribal Members the importance of keeping your addresses updated. This helps to prevent delays in receipt of correspondence (checks, newsletters, other information) that is mailed by the Yavapai-Apache Nation. Power of Attorney forms are encouraged for those requiring individuals to act on their behalf (only in matters pertaining to Enrollment such as checks, attaining documents or updating information). The Enrollment Department now requires a Check Pick-Up Request Form for all disbursements being picked up by someone other than the recipient. Verbal/phone requests will not be accepted.

All forms can be found by visiting the Yavapai-Apache Nation website at the link below or by coming into the Enrollment Office.

The deadline to submit check pick-up requests is the day before disbursement no later than 5:00 pm. Check Pick-Up Request Forms will not be accepted on disbursement day. Any remaining checks will be delivered by certified mail the following week. As soon as the Enrollment Department is notified of the date of disbursement, the dates will be posted on the website. Please check back for these dates.

Your assistance with this matter is appreciated! If you have any concerns or further explanation is needed, do not hesitate to call or send us an email.

Thank you,
